Have your insurance card handy and our team will check whether your plan is accepted.
2. Check with your plan
Call the number on the back of your card to confirm your benefits and any referral requirements.
3. Bring your card
Bring your insurance card and a photo ID to every visit so we can keep your information up to date.
Billing
Copays and balances
Copays are collected at the time of your visit. If you have a deductible or other balance, we’ll let you know and explain it clearly.
