New patients
Getting started is simple
We’re glad you’re considering P & P Health Center. Here’s what to expect, from your first call to your first visit.

Three easy steps
How to become a patient
1. Reach out
Call us with any questions. We will begin scheduling appointments in the first quarter of 2027.
2. Complete your forms
Fill out your new patient paperwork ahead of time so your visit starts smoothly.
3. Meet your doctor
Bring your ID, insurance card and medication list. We’ll take it from there.
What to bring
Your first visit checklist
- Photo ID
- Insurance card(s), including Medicare and any secondary plan
- A list of your current medications, vitamins and supplements, or the bottles themselves
- Any recent lab results or records from your previous doctor
- Your completed new patient forms
- A list of questions you’d like to ask
Your first appointment
What to expect
Please arrive 15 minutes early. Your first visit is a chance for Dr. Barciela to get to know you: your health history, your goals and anything that’s been on your mind.
We’ll review your medications, update any screenings you’re due for, and agree on a plan together. If you’d like records from your previous doctor, we can help request them.
Paperwork
New patient forms
Completing your forms before your visit saves time at check-in.
Your packet also includes an optional form about clinical research. Whether you sign it or not is completely up to you and never affects the care you receive.
